Why did this item not appear as paid in my "Items to be shipped" section?

He did not complete checkout granted, but he paid via PayPal and did include the item number. He claims he never can complete checkout.
What was the problem here? It's embarrasing not only not to ship an item in a timely manner, but to tell the buyer he hasn't paid when he has!
