Decided to look over my account and noticed that it says "Insertion fee. Relisting item. Default" on NEW items that I listed, meaning they are NOT relists at all. Does anyone have that in their account and exactly what does it mean?

Don't worry, it's just a glitch (tm).

It started sometime around Wednesday or Thursday; around one in three auctions get listed like that. You'll notice in your listing confirmation email that it says that your listing fee will be refunded if the item sells. It won't happen though; I just ended an auction that sold, and I didn't get a refund.
