I am pretty new at selling, and in my description I put all sales are final. I state the size, the measurements and everything. I just got an email that said "the pants don't fit" She says her waist is the size in my description, WHAT DO I DO? Should I give her a refund? How does a refund work? Do I send the refund right away, or wait til I receive the item? Do I refund shipping charges? PLEASE someone help me.

Welcome to ebay and Buyers remorse. I would refund her money, but only AFTER, I received the pants back, and had checked them for any problems. It's unfair, but it's the best road to walk. Sorry.

Even though you said all sales are final, if you are a relatively new seller, you probably want to avoid the possibility of getting a negative feedback so a refund would probably be the way to go.
Do not, however, send any refund until you receive the item back and inspect it to ensure that it's the one you sent out and it hasn't been damaged in any way. The buyer should pay postage to return the item to you since you stated the measurements in the description and you should only refund the bid price. You should not be out any money if your description contained all the size info and they bid anyway.

I don't sell clothes but I have made them and I also knit. The clothes measurements and the body measurements are two different things. Unless they want the clothes to fit skin tight they have to have 1" to 2" or more, "ease" in the clothing to fit comfortably. In other words, a 30" waist measurement in the pants would be very tight on a 30" body waist.
Perhaps you should say something like "allow for ease in all measurements" in your discriptions. A lot of people don't understand this.
