There isn't one that I know of... what are the circumstances? Why did your customer ask you to do that?
If you filed a NPB alert or FVF's and then received payment from the buyer (or realized you made a mistake) all you have to do is email [email protected] and let them know the buyer paid and they will remove the warning from that buyer's account.

Hello,
As far as I know there is no NPB removal. But they do have an online final value fee removal form. I found this out last week after I wrote to billing to have a fvf removed after receiving payment. Here is the link they gave me http://cgi3.ebay.com/aw-cgi/eBayISAPI.dll?RemoveNPBWarningShow

Leave it to eBay...did you notice this is called REMOVE NPB WARNING, but you can't use it until after you have filed for and received FVF credit?
That's as backward as the technique for filing for FVF because you gave a refund! You have to send a NPB form and check it's for a refund. The wording is confusing, too, because it ask if you received money. Well, yes I did but I returned it....However, you must say NO. Then the buyers gets an email and are told eBay has been informed they decided NOT to pay for item!!!! Sometimes, the bidder is not happy. Now, I know to let the buyer know ahead of time this is just how eBay does it.

I would never use such a form. If payment hasn't arrived on day 10 after doing an alert, the bidder is history. I'd file for credit and relist. They can cry and stomp their feet all they want.
This is just another way eBay gets to make sellers do extra work and permits bidders even more time to delay making payment instead of hastening their departure via NARU status.
Maybe eBay needs a refresher course in basic business. You buy something, so you pay for it. How hard is that?!?!

Actually, it does make sense. The crediting of the FVF to the seller is what generates the "black mark" on the bidder's account - until that point, there isn't anything to remove. And the first email from eBay is called an NPB Alert; the second one is the NPB Warning, so the form is titled correctly.
