In Global Preferences I have US Insurance set to Optional and cost set as "using calculated rates specified below" which defaults to "(calc)".
In SM Pro 7.1 the program will not let me save an item unless I enter an amount in the insurance box.

Hi there,
I believe this is a bug, and we have notified engineering about the problem. In the meantime you should be able to work around this by entering a fixed amount.

And what happens if there are multiple purchases? Will they be charged for insurance on each item? Or will they combine? Most of my buyers pay long before I can send an adjusted invoice, that is why I take credit cards.

If a buyer combines, it gets charged on each item (we have an open enhancement request to have this behavior changed). If you are combining for the buyer, you will get the option to choose shipping for the order as a whole, which means insurance applies to the shipping for the entire order as a whole rather than individually.
