Okay...let's get real...
-Massage, foot, body, spa package. Forget the facial, emphasis on the relaxation aspect of things.
-do not buy an appliance, unless it's from
www.goodvibes.com
-Cashmere sweater set can never do a woman wrong
-Set of nice pearls, depending on your budget. Get nice round, white with bluish tint, look much better on most skintones. Mikimoto never did a girl wrong. Develop a relationship with your local jeweler, not McJewelry store. Your local guy/gal will care enough to get the perfect item.
-Run screaming past the cheap perfume sets.
-Do consider the things she talks about, cares about and loves. Meditate upon them and a gift will emerge.
Those who love you and whom you love (did I do that whom, who thing right?) will listen to the inner giftee talking.

Japerton: Some great ideas in your list, and, yes, your "whom" is perfectly used.
I'd forgotten about pearls. Some of us don't wear them often, but sometimes they are the perfect accessory. I have a short necklace and an opera-length necklace. I have button pearl earrings, 2 sets, but I wish I had some that dangle a teeny bit. I may have a jeweler make over one of the button sets.

The Victoria Secret lotions are nice... but can't compare to the joy she'd get showing you some tough love.. with her little whip!

micmic, here's a good one, no shopping involved, & you can even get in on it too. 
A pretty card, put a nice bow on the envelope, & inside write a note saying something like "Good for a night out, dinner at your favorite restaurant & a movie."

Diamond Stud Earrings (of a decent size 1/4 carat each and UP) are ALWAYS the perfect gift.
I had some, lost one. So my next pair will have to have screw on backs. Had a nice necklace made out of the one I had left.
My favorite all time gift I've gotten? My husband sucked my toe one morning to find out my toe size and then got me a gold wedding band toe ring for Valentines Day. I had no idea what he was doing!

Ah, women are so hard to buy for. I was really in a quandry as to what get my sweetie, but she solved the problem a week ago Sat evening. We were watchin TV & an ad came on for computers showing the liquid display monitors. She said, "I would really like to have one those." (She is a medical transcriptionist working at home) Well, Monday I hopped down to Circuit City & got her a 17 inch monitor. I took it up & installed it for her. OH MY is she happy!!!! So am I. LOL
