(never happend to me before) -- I've sent three emails with no responses for a $5 item. What irks me is that "he/she" has 10 positive comments (with one of them happening in the last 7 days!) It seems very un-characteristic of the person to get a positive remark on a purchase while ignoring my emails within the same time period.
How brutal should I be in the feedback?
Negative: Agreed to winning bid, then never responded to emails.

Did you send out the NPB? Sometimes that gets their attention. You should do it anyway to clear the path to sending out the fvf credit. These are the things that alerts ebay that this bidder may be a problem. But I'd do atleast the NPB first before leaving the feedback. Maybe they will come clean and you won't have to proceed.

Have you tried using the "Send this Auction to a Friend" from the auction page? Have you tried sending from a free email account (i.e., Hotmail or Yahoo)?
If not, you might want to try this. Many times one end or the other is rejecting or dropping the email because of a spam filter. The buyer may NOT be getting your email.
Of course this brings up the question of why the buyer has not contacted you (if it has been three days).
You may also want to pull the contact information. This may prompt the buyer to wonder why and email you.

I agree with Greg and Nanastuff. I would try emailing him/her from a different email address and be specific that you expect a response by email. Some bidders will just drop a check in the mail without responding. At the very least, give it a week or ten days to see if payment shows up.
