Send them back the second payment minus fees.
With luck, someone else can confirm that there is some way to cancel a payment and not get charged fees.

If you are sending you won't get charged more fees. The buyer will but that's the cost of his/her mistake I guess...I'd do like captainkirk said and send the balance of what you got minus the fees.

You can call PayPal at 888-221-1161. Tell them you received a duplicate payment and that you are (or have) refunded the duplicate payment and you would like a credit for the fees. They will credit your account for the fees on the duplicate payment you received and refunded. Everyone should do this regardless of how small the fees until it gets to be such a big pain for them that they will provide a facility for duplicate or overpayment refunds.

One thing - forgive me if this seems bloody obvious - but a few times I have received two separate copies of the paypal notification email, but only one payment was made. Double check that they really paid twice before sending money back!
