Okay, I know something like this exists. I just can't find it anywhere.

I have a customer that claims they did not receive their package. It was a lightweight small ticket item shipped first class without tracking or insurance.

Isn't there a USPS "missing mail form"? Does anyone know the correct form name and form number?

what exactly is that form good for? can the receiver of the item fill it out or is the sender supposed to?

I think it's the sender who fills it out.
