I've never done one with PayPal, but with a credit card, you would call the credit card customer service number, explain to them why you don't think you should pay the charges (e.g., never received, broken, fraudulent charge, etc.) and they deal with the merchant. They will probably ask you to send them a written summation, either through email or snail mail.
FWIW, the one time I felt to a moral certainty that I deserved a huge refund, I did not get one. My wife's Amex was the card, and I told her it was one more reason not to use Amex. Amex basically hid behind legal definitions of what constitutes delivery of travel.

If the charge back is with paypal, what I did was call paypal and tell them I did not authorize that payment to be taken from my account, which was the truth.
Because when I called the company they told me that it was written in the fine print, which I told them there was no fine print.
Well I filled out a complaint with paypal and within a week they credited me back my money, which the company told me I had no choice but to pay it.
You will need to fill out a form that is in the paypal site and mail it into to them, but call them first the girl will tell you what needs to be done.
