I did market research for a living, and I have to tell you I hope you didn't spend too much to have that survey prepared, and that you don't plan to take any actions based on many of the questions!
Every question required an answer, and yet many questions did NOT have "none of the above" (or something like that) for a choice.
When's my busiest revenue day? I have 3. But I could only choose one. What if I didn't know? I couldn't tell you that ... I had to pick one, so my answer (and many other people's) will be arbitrary.
How am I planning to expand my business? Not by any of the choices I was given, and yet I had to give you an arbitrary answer.
Scrap the survey, folks ... you aren't going to get accurate data from about half the questions.

Late to the topic, but I couldn't agree with you more OP. I got about halfway through and just stopped. Too many variables that require an OTHER answer along with a brief explanation.
The survey was worthless, especially if you're looking for a good cross-section of answers.
